Mèli Youth Games (Ages 10+)

Consent to Participate

I am the parent or legal guardian of the child named above and give permission for my child to participate in supervised youth game activities hosted by Mèli.

Program Eligibility

I confirm that my child is 10 years of age or older and able to participate in a structured group activity, follow instructions, and regulate emotions.

Description of Activities

I understand that activities may include tabletop games, group play, collaborative problem-solving, strategy games, and age-appropriate themes conducted indoors in a group setting.

Assumption of Ordinary Risk

I understand that participation in group activities may involve ordinary risks, including minor physical injury, emotional frustration, or interpersonal disagreement, and I voluntarily assume these risks on behalf of my child.

Medical Authorization

In the event of a medical emergency, I authorize Mèli staff to seek appropriate medical treatment for my child if I cannot be reached.

Code of Conduct & Removal

I understand that my child is expected to follow program rules and staff direction, and that repeated disruptive, unsafe, or inappropriate behavior may result in removal from the activity without refund.

Pick-Up Acknowledgment

I agree that timely pick-up is required. A $10 late fee may be assessed for pickups occurring more than 15 minutes after the scheduled dismissal time.
